Please complete the following steps to complete your BCI (FBI Background Screening).

1. Print and fill out your BCI Application. Complete all highlighted sections, for Host Home, 1 per adult (18+years) living in the home. 

2. Scan and email your application and a current photo ID to info@cryouth.com.

3. The cost of your BCI is $57.25. Please contact 801-602-4039 for payment info. *45 days after your first client placement or DSPD hire date you will be reimbursed for your BCI fee.

4. You'll receive a Live Scan Fingerprinting Authorization Form from Crossroads, please print this form, you'll need this form to schedule your Live Scan.

5. Last, schedule with YUP Finger Printing and find a location near you. You can schedule your appointment online, You will select "Direct Submission/Livescan". 

Please print your Authorization form, bring a copy of your social security card and photo ID to this appointment.

Once all of the above steps are completed your BCI application can take anywhere from 2 to 6 weeks to process, Crossroads will notify you when we receive confirmation of approval or denial.
