Guidelines and Policies

Welcome! Here are a few guidelines to our Day Center that will help you and us offer better support to our clients!

1. We may be closed on specific Holidays

▪ Please visit our website for a list of current closure date. Please plan accordingly.

2. Individuals are responsible to bring their own meals

▪ Please note, we do not provide breakfast or lunch.

▪ If for any reason Crossroads does provide a meal/meals for an individual, we do charge a $5 fee for each meal provided. Crossroads will invoice caretakers at the end of the month.

The charge has been communicated to individuals at the beginning of the year. It is the caretaker’s responsibility to communicate the charge with the client thereafter.

3. Adult briefs are required for any individual that requires assistance with toileting

▪ Individuals are required to provide their own adult briefs and wipes.

▪ Individuals are required to bring 2 extra pairs of clothes for any toileting accidents. It will remain at the day center and be returned unwashed when soiled.

4. Activity fees must be paid by individuals

▪ Our activity calendar is on cryouth.com.

▪ Crossroads will pay for the activities upfront and then invoice individuals/ caretakers at the end of the month. ▪ Receipts are available upon request.

▪ Accepted forms of payment: online via card, direct bank transfers, or check. ▪ Not accepted forms of payment: Venmo and cash.

▪ Invoices are due within 48-72 hours upon receiving it.

▪ Please give us a 48–72-hour notice if you prefer your individual to stay back from a specific activity-due to preference or not being able to pay for it.

5. Sick Protocol

▪ Individuals must not attend day center if they are experiencing any symptoms of the flu.

  • This includes, but not limited to, a fever of 100.4 F or higher, vomiting, diarrhea, rashes, lethargic. Caregivers are required to pick up the individual from day support.
  • They are allowed to return to the day center 24 hours after being symptom free.

6. Guardians/Caretakers may be required to pick up individuals if they are having behavioral or health issues.

7. We have a right to suspend or terminate services if an individual becomes aggressive towards other clients or staff members.

8. Transportation

Routes & Times

South County (Orem & Provo) pickup starts around 7:55 AM, with drop-off starting at 2:10 PM.

North County (Lehi, PG, N. Orem) pickup starts around 8:20 AM, with drop-off starting at 2:40 PM.

Times may vary due to traffic or road conditions, so please allow a 15-minute grace period.

Pickup Guidelines

Please have individuals ready within 10 minutes of the driver’s estimated arrival time.

If the individual is not ready within 10 minutes of the driver’s arrival, the driver will notify the caretaker and continue the route. In this case, the caretaker will be responsible for transporting the individual to Day Support.

Drop-Off Guidelines

A caretaker must be present at home or facility when the client is dropped off.

▪ Exceptions must be approved in writing via teuila.crossroads@gmail.com.

Tip: Keep an eye out for our van to help pickups and drop-offs stay on time.

Transportation Fees

If caregivers are not at the individual’s residence within 10 minutes of the scheduled drop-off time, the route will continue, and the driver will complete the remaining drop-offs. Caregivers will then be responsible for picking up the individual from Day Support at 538 Commerce Rd, Orem, Utah 84058 after the route has been completed.

Please note that the arrival time back at Day Support may vary depending on traffic and remaining routes. Drivers will stay in contact with caregivers to provide updates.

If this occurs more than once, caregivers will be charged a $25 fee, plus an additional $20 per hour for the time the individual is held at the Day Center. Crossroads reserves the right to suspend transportation services for the individual.

9. Snow Day/Delays. Snow Delay is when the road conditions are unsafe to travel on, due to ice or snow.

▪ A Snow Delay is determined by 7:00am. All clients will be notified by text message of the delay.

▪ It is decided by 10:00am if the roads are clear and safe to start pickups.

▪ Pickups will start at 10:00am or sooner if roads are clear. All clients will be notified by text message. ▪ Drop-offs are also delayed. Caretakers will be notified immediately of the estimated delay time.

▪ Field Trips/Outings will usually be canceled on these days.

10. Day Support Individual (DSI) Staffing and Service Policy

▪ If an individual receives 1:1 Day Support Individual (DSI) services and their assigned staff member call out, we will make every reasonable effort to find another staff member to provide coverage.

▪ If coverage cannot be secured, Crossroads reserves the right to cancel DSI services without notice. While we will always strive to notify families or caregivers as soon as possible, but circumstances may arise that prevent advance notice.

▪ Our priority remains the safety, consistency, and quality of care provided to each individual we serve.

11. Each individual/ legal guardian must have a signed Waiver of Liability / Release of Liability on file.

▪ This waiver protects both the individual and our program and is a required condition for the individual receiving services from Crossroads Day Support.

▪ The waiver will be sent in a follow up email. Acknowledgment & Agreement.
